With over 25 years of experience, we support adults aged 18 and over, including people who live with dementia, or have multiple needs including physical disabilities and mobility needs.

DEMENTIA SUPPORT

Safe, high quality care and support for people living with dementia and their carers.

Our aim is to help people with dementia live as independently as possible in familiar surroundings while remaining active, connected and involved in their community. We recognise that every person’s experience of dementia is unique, and our support is tailored to their individual needs, preferences and life history.

Care 24-7 is affiliated with the Contented Dementia Trust and our staff are training in The SPECAL Method. This approach helps our team understand the experience of dementia and provide support that promotes wellbeing, reduces distress and maintains dignity. In addition, our staff receive dementia training that is aligned with recognized dementia framework standards, ensuring they have the knowledge and skills to provide high quality, person-centred care.

Types of short breaks available

We offer flexible support to meet individual needs including:

Short Break Visits

Usually provided in blocks of 4 hours or more

Daytime, evening, weekday or weekend support

Overnight Support

Sleep-in support where appropriate

Overnight waking night care

24-Hour Home Care

Around-the-clock care and support

Inclusive of sleep in provision where required

Individuals with a Care Act Assessment of Need via Stockport Council

People receiving Direct Payments

Individuals who care is commissioned by Stockport Council

Getting started with Care 24-7 is simple, friendly and focused entirely on you.

When you get in touch, we’ll take the time to listen and understand what kind of support you and your loved one need.

We’ll then make sure everything is safely in place, carrying out any necessary checks so you can feel confident and reassured from the very beginning.

From there, we’ll create a personalised support plan together and carefully match you with the right care team. Once everything is agreed, your short break support can begin

We’ll stay in touch throughout to make sure the service continues to feel right for you and adapts as needs change.

Charges

Once your personalised support plan has been agreed, we will confirm the cost of your care so everything is clear from the start. We will always make sure you understand the charges before your support begins, with no hidden costs. Our team is happy to talk you through the options and answer any questions you may have.

Transportation

We do not provide transport. However, we can offer information and advice about local transport options and will work collaboratively with local transport providers to help ensure planned activities and community access run smoothly.

Our Stockport service is currently awaiting its first CQC inspection.
